Job Requirements 

Occupational Therapist / OTR or COTA
  

 

* Active Indiana Occupational Therapist or COTA license.

* Current CPR certification.

* Two step PPD within the past year or negative Chest X-ray.

* Physical Exam within the past year.

* Three professional therapy references.

* Must pass a company-paid Criminal Background Check and Drug Screening.
* Professional appearance and strong work ethic.

* Excellent communication skills.
