Full-time, weekend RN needed at our home health care agency in Albany. This is a field position requiring local travel to patient's homes. RNs with Home Health, ER and ICU experience preferred. Saturday, Sunday and Monday schedule available.

We offer terrific pay rates, full benefits and mileage reimbursement.  This position is responsible for the planning, coordinating and delivery of quality skilled services.

Facilitates communication and coordination of care between clients, families, caregivers, physicians and other service providers as necessary.

Assures current and accurate clinical records in compliance with documentation requirements and program specific rules for coverage and qualifying criteria.
