Case Management Nurse Consultant

POSITION SUMMARY
Our Case Managers play a key role in impacting members lives by providing coordination of care between key providers of the member s health care team. Responsibilities include: managing cases across the continuum of care to ensure a positive patient outcome, evaluating medical necessity of care, developing an understanding and implementation of specific client requirements and educating members about appropriate community resources, patient rights and safety. You will also facilitate engagement with members to discuss and implement plan of care needs and will monitor the developed plan of care.

Candidate agrees to work a minimum of one 8 hour evening shift per week upon successful completion of orientation period.

Requirements LICENSES AND CERTIFICATIONS

Registered Nurse (RN) is required
