Bilingual Nurse Needed.
VillageHealth is looking for a bilingual Spanish/English RN to join an integral part of VillageHealth's most exciting and meaningful new health initiatives. VillageHealth provides disease management services, coordinating comprehensive, individualized care plans tailored for the specialized needs of patients with renal disease.
This key role is based in the Riverside, CA focusing on patients living in this area. This position functions autonomously and in collaboration with all members of the healthcare team to coordinate and facilitate quality, cost-effective care, while minimizing fragmentation of the healthcare delivery system for ESRD and CKD patients. This position provides a link between the patient, family, physician, provider and care teams, the community, and Regional Operations Manager.
Core duties: - Identify and assist with the needs of patients including, but not limited to medical, social, emotional and financial concerns through medical record review, consultation with patient, family and health care team members
- Coordinate with physicians and health plan by obtaining appropriate referrals for members, appointments, and providing updates as necessary
- Provide patients, families, physicians, hospital staff, and DaVita's Revenue Operations department with information related to insurance information coverage
- Evaluate, coordinate, and facilitate patient/family access to necessary and appropriate health, human, and community services
- Help patients achieve an optimal level of function by empowering patients to direct their care, self-advocate, and make decisions whenever possible
- Implement innovative nursing interventions appropriate to the long-term needs of the patient, which complement and support the medical treatment plan, while integrating psychosocial, physiological and discharge planning needs
- Apply nursing process (assessment, planning, intervention, and evaluation) in managing caseload
- Use relevant data such as history and physical, lab data, discharge summaries and access procedures with information obtained through the interview process and assessment of the member to coordinate care for the member
- Enhance patient satisfaction and retention through continuous engagement and follow-up of care and care plans
- Coordinate health care services to ESRD and CKD members across the continuum of care, including but not limited to physicians, emergency services, inpatient acute and chronic facilities, mental health and home health facilities, home and extended care, ambulatory care, and hospice in continuing efforts to contain costs, reduce hospitalizations, decrease length of stay and reduce the number of hospital re-admissions
- Identify and redirect practice patterns related to over-utilization of services, under-utilization of services, premature discharge from appropriate level of care, permanent or temporary alterations in functioning, medical/psychological complications, and insufficient patient education of disease course/process
- Identify all existing and required clinical services in each market area
- Uphold patient Bill of Rights and right to confidentiality
- Know and comply with:
- DaVita policies and procedures in accordance with state and federal regulations for patient care and services (CMS, HIPAA, OSHA, or other governing agencies)
- Standards necessary to maintain organizational accreditation (such as NCQA)
- State or federal regulations of the Injury Prevention and Safety Training Program
- Other duties and responsibilities as assigned
- Flexibility to adjust schedule to accommodate organizational and/or customer needs with limited notice
- Attend staff meetings, phone conferences, and training as needed
- Know, understand, and follow teammate policies and procedures
- Maintain and meet all annual competencies and skills requirements
- Travel Required: up to 75% as required for position (local travel within the area daily/weekly; 1-4 trips outside CA annually as necessary for training)
Requirements
- MUST HAVE BILINGUAL LANGUAGE ABILITIES IN ENGLISH AND SPANISH.
- A.D.N degree from accredited school of nursing required; B.S.N preferred; three-year diploma from accredited diploma program may be substituted for nursing degree
- Current RN license in the state of California required; continuing education credits maintained as required by state of practice required
- Minimum of five (5) years' experience in clinical nursing required
- Minimum of three (3) years' experience in renal nursing preferred
- Certified Nephrology Nurse (CNN) or Certified Case Manager (CCM) preferred.
- Current CPR certification required
- Functional proficiency with DaVita specific clinical software programs required within 90 days of employment required
- Basic computer skills and proficiency in MS Word and Outlook required
