    Disease Managers

    Disease managers are passionate about health education; this position is your opportunity to encourage people to engage in disease prevention and screening activities. Nurses use their communication, background experience, and advocacy skills to motivate their members to improve their health. Responsibilities include: managing a caseload of patients in a telephonic disease management program, identifying needed medical services, modifiable risk factors and educational needs and implementing a personalized disease-specific action plan. You will facilitate communication and collaboration among members, family, provider and other health care team members, educate and reinforce appropriate self-care, and help members actively participate in their own health care decision-making with their provider.

  • Provide members with health care information to drive member understanding of his-her health condition(s) and possible options (Mastery)

  • Present cases at Medical Director rounds and case conferences (Mastery)
  • Refer members to alternative healthcare programs (i.e. disease management, case management, vocational rehab, etc.) (Mastery)
