Provides psychosocial assessment of and brief targeted intervention for social, emotional and environmental issues that impact client/family ability to optimally benefit from care. Provides the social work component for case management triad, actively collaborating to meet patient care, outcome management and system improvement goals. Populations served may include infant, pediatric, adolescent, adult and geriatric patients.Strongly prefer Outpatient Oncology experienceMinimum Education: Master’s Degree in Social Work, Possess LSW
