Diabetes Clinician The Diabetes Inpatient Clinician is responsible for implementation of assigned program aspects that address objectives of the company and the customer. This position requires a committed individual who demonstrates a systems approach to management. The Diabetes Inpatient Clinician will translate business knowledge into behaviors that produce high quality, cost-effective outcomes. This individual is able to apply program evaluation and accepted clinical research findings to secure clinical and program outcomes. He/she will build strong customer relationships, actively explore customer needs, and pursue creative solutions to satisfy those needs. Responsibilities Include: 1. Works with the Program Director, physicians, Center staff and other hospital staff to achieve program objectives and enhance clinical outcomes and customer satisfaction. 2. Participates in the marketing and media plans established for the Center. Represents the company to physicians, local and national organizations, and community groups. 3. Actively participates in and integrates current hospital structure, policies and procedures, and forms, as related to diabetes care. 4. Based on program objectives, and with guidance from the Program Director and the Clinical Department, determines appropriate methods to measure, analyze, and interpret results to clinicians and administrative staff. 5. Establishes Center specific outcome measures for aggregate clinical diabetes populations (as the elderly, those with specific chronic complications, pregnant, etc.) 6. Establishes a Center specific system for understanding and communicating effectively with physicians involved in diabetes care and for assessing physician satisfaction with diabetes service line. 7. Mentors other clinical staff members in Outcomes Management behaviors. Clinical Responsibilities: 1. Available as a resource to program staff, nursing, and ancillary department staff. 2. Ensures that all components of the program meet the Quality Assurance Standards as established by the company and the hospital. 3. Participates in Quality Assurance Advisory Committee meetings and contributes to yearly program review. Participates in other hospital team meetings as necessary to ensure hospital-wide continuity of care. 4. Assists in orientation and development of diabetes treatment team as required. 5. Reviews case variances and outcomes to improve organizational and clinical systems of care. Performs variance coding and tracking on a case load of 20-30 patients at any given time. 6. Supervises case management of complex diabetes cases through Clinical Nurse Diabetes Inpatient Clinicians, Social Workers, and other clinicians. 7. Facilitates the process for development and implementation of the Outcome Focus Teams. 8. Modifies or develops practice guidelines, Standards of Care, and Policies and Procedures, etc. 9. Establishes a system for providing a follow-up care for target diabetes groups. Financial Responsibilities: 1. Explores strategies to maintain and/or reduce resource consumption; to reduce length of stay; facilitates implementation of these strategies house-wide; and to monitor results. 2. Supports the Center's physician marketing activities. 3. Participates in data collection and analysis to meet the program evaluation system objectives. 4. Utilizes the value cycle with all customers on an ongoing basis. 5. Participates in translation of outcome management data into cost management terms.
1. Registered nurse with a minimum of a Bachelor's degree (Master's preferred) with previous two years experience focused in diabetes care. 2. Current state license. 3.
