- Masters Degree- ~CB~- Social Worker- Registered in facility state

Social Worker

 

Job Responsibilities:

  • Provide psychosocial counseling, information referral and treatment planning for patients and their families to maximize the social functioning and adjustment of ESRD patients
  • Work with a team of clinicians in developing and monitoring short and long-term care plans
  • Conduct psychosocial assessments and counsels patients on life style issues related to ESRD
  • Act as an advocate on behalf of the patient and family
  • Provide patient education and/or support groups
  • Participate in the care center's QA/CQI process

 



Job Requirements 

 

Education:  Masters Degree in Social Work (MSW);

 

Experience:  Minimum of two years of experience as a social worker in a health care setting, dialysis exp preferred
