Patient Safety Manager
Job Responsibilities:
Works closely with MTF leadership to promote a culture of safety throughout the MTF and formulate policies that effect safer care. Make recommendations for organizational changes. Administers and manages day-to-day Patient Safety (PS) program operations encompassing all aspects of inpatient and ambulatory care, and support functions. Responsible for coordination of root cause analyses (RCAs), failure mode and effect analyses (FMEAs), and other PS initiatives brought about by the event reporting and proactive risk assessment functions of PS Program. In order to carry out the functions, the PS Manager shall have access to all necessary and relevant MTF and medical staff data, including but not limited to committee minutes, medical records and medical staff files. Ensures mechanism is in place to track deployment and sustainment of risk reduction strategies and improvement recommendations resulting from PS activities. Communicates status in writing to medical treatment facility leadership and staff at least quarterly, and includes a summary of status in the annual report. Is a key member of the organization in preparation for Joint Commission (TJC) and / or Accreditation Association for Ambulatory Health Care (AAAHC) survey.
Specific Tasks:
1. Plan, develop, implement, and coordinate Patient Safety functions in accordance with AF and DoD directives.
2. Serve as Patient Safety resource and confer with all levels of personnel to develop and direct the program.
3. Conduct an annual appraisal of the adequacy of organization-wide Patient Safety activities/policies/procedures to ensure program effectiveness.
4. Collaborates closely with all other facility functions on items/issues related to risk indemnification, risk assessment, and risk control.
5. Ensure all events are reported according to AF and DoD instructions.
6. Collect, collate, analyze, and display data from facility event reviews, RCAs, FMEAs, and other sources.
7. Collaborate with the facility risk manager in managing daily event reports.
8. Collaborate with the facility process improvement manager on the use of analytical tools.
9. Develop, coordinate, and present ongoing Patient Safety education in the form of new employee orientation, annual recurrent training, in-service training to all personnel and one-on-one consultations as necessary.
Required Education:
1. A Bachelor's degree in nursing , health care administration or equivalent clinical health care degree
2. If in a licensed healthcare profession, licensure must be current, valid, active, and unrestricted.
Required Work Experience:
1. Hands on experience in Microsoft Access, Excel, Visual Basic, and SQL with experience in relational databases and visual report design tools. Experience with quantitative and qualitative data analysis and data manipulation necessary. Proficiency with statistical packages a plus .
2. Knowledge of The Joint Commission (TJC) and / or Accreditation Association for Ambulatory Health Care (AAAHC) standards and scoring guidelines desired.
4. Must possess an understanding of and experience with performance improvement tools, which include, but are not limited to, brainstorming, flowcharting, meeting skills/team dynamics, multivoting techniques, data analysis methods, etc. Experience with root cause analysis (RCA) and failure mode and effect analysis (FMEA) highly desired. Proficiency in Microsoft Word and PowerPoint required, and Microsoft Access and Excel desired.
5. Must possess ability to work, organize time, and manage multiple projects simultaneously. High standards of excellence and attention to detail required. Effective writing and presentation skills required.
6. Military healthcare experience a plus.
