Patient Referral Rep

Description Shift:  Evenings

Scheduled Work Hours:  8:00 to 5:00

1.Initiate contact with Health Care Provider (HCP) clients by phone and fax2.Mail information packets to HCP contacts3.Develop and maintain good communication with the office manager, head nurse, or other appropriate person in each HCP office, with the goal of generating recurring business4.Further develop a rapport with each office contact person by offering 'hands-on' problem solving for their individual patients5.Transition HCP referrals to LMS profiles, as most referrals are newly diagnosed diabetics6.Support a 'team' concept within the department as far as sharing helpful information that may be forthcoming from office contact persons, as well as when assisting an office contact from a state other than the rep's assigned state7.Support Territory Managers in establishing and maintaining physician relationships and in the enrollment of patients through Territory referrals for both Medicare and commercial plans.8.Adhere to all departmental requirements, i.e., placing an appropriate volume of calls, maintaining complete and accurate log sheets, assembling information packets as needed, and maintaining contact information in the Goldmine system9.Generate leads to LMS Patient Enrollment Specialist and, once a profile is established, notify the specialist who initiated the lead10.Maintain and update established accounts on a regular basis11.Stay informed as to any changes in Medicare/Medicaid coverage requirements12.Participate in ongoing product training and take advantage of any information distributions that will enhance the ability to perform at a high level13.Additional duties required as directed by the Patient Referral Manager.Liberty is a Drug Free Work Place and an Equal Opportunity Employer.
