Description GENERAL DESCRIPTION OF DUTIES: (an employee in this classification will be responsible for, but not limited to, the following tasks)

An employee in this classification is required to perform some or all of the following duties, however these do not include all of the tasks which the employee may be expected to perform: complete and document comprehensive nursing assessment of service recipient; maintain accurate and timely service recipient records; develop goals and objectives in cooperation with service recipient, service recipient’s family and/or guardian, and other team members for appropriate plan; provide direct nursing services to service recipient; consistently implement and follow Person Centered Planning process; provide observation and monitoring of nursing service/goal implementation and recommend improvement opportunities; coordinate/link with physicians within the community; provide training/education/consultation to service recipients, staff, and/or family/guardian to support Individual Plan of Service; maintain appropriate, accurate and timely documentation as determined by the Agency; assist in monitoring of medication effectiveness, and assisting with enabling service recipients to obtain/understand medications, during clinic, program or home contact; provide after hours interventions (urgent needs) when needed; and to maintain confidentiality.
Requirements MINIMUM QUALIFICATIONS: (an employee in this class, upon appointment, must have the following credentials, training, and experience)

Technical Skills
 Education:
 Graduated from an accredited Nursing Educational program
 Licensure:
• Active Registered Nurse License from the State of Michigan
• Valid Michigan Driver’s License
 Experience/Skills:
• Knowledge of Current Nursing Practices and Methods
• One to Two (1-2) Years Experience in Job Related Field
• Demonstration of Ability to Use Word Software and Windows XP Programs
 Other:
• Must Have Access to Transportation
• If Using Agency Vehicles, Passing Random Drug and Alcohol Screening

 Education:
 Bachelor and/or Master degrees preferred
 Experience/Skills:
• Three to Five (3-5) Years Experience Providing Nursing Services to Mental Health Individuals who Receive Services
• Demonstration of Proficiency and Experience Working on an Electronic Health Record System
• Knowledge of CMH System and Operations
