Position Description:
The American Cancer Society is the nationwide, community-based, voluntary healrh organization dedicated to eliminating cancer as a major health problem by preventing cancer, saving lives, and diminishing suffering from cancer through research, education, advocacy, and patient services.
Responsible for guiding patients and their families through the cancer care continuum within University Medical Center and Southwest Cancer Treatment and Research Center to identify barriers to treatment and assist with access to needed resources to decrease these barriers. This includes connecting patients to and raising awareness about the American Cancer Society information resources, support programs and services, and assisting in meeting other cancer-related needs articulated by patients and families.
RESPONSIBILITIES
Help patients move through the complexities of the healthcare system by assisting with practical problem solving related to concrete needs such as lodging and transportation, and other services that decrease barriers to treatment.
Manage implementation of ACS programs at UMC, e.g. on-site visitation programs, support groups, etc.
Promotes ACS programs, services, and volunteer and partner opportunities including: Information/Referral using ACS online/toll-free patient service center. Referrals to direct services provided to patients and caregivers through ACS and other community partners, management of ACS Resource Center at UMC, identifaction of patient/family caregiver resources and needs, and management related community resources database content.
Responsible for establishing an ongoing relationship with the health care community and UMC.
Will assist in recruitment, interviewing, training and supervising ACS volunteers for on-site programs.
Will enhance and maintain the Community Resources Database.
Develops strong partnerships with UMC's management and staff and ensures that staff are aware of and participate in, when appropriate, ACS activities.
Invites cancer patients and their caregivers to become part of the cancer survivor community through such venues as ACS Cancer Survivor's Network and Relay for Life.
Works within UMC to promote navigation and build patient referrals.
Works in partnership with Regional Director Health Initiatives to provide necessary information for reporting and goals.
Performs other duties as assigned by Regional Director Health Initiatives.
Position Requirements:
KNOWLEDGE, SKILLS AND REQUIREMENTS
Bachelors’ degree, Master's preferred (both in Social Work) with current state licensure/certification.
Minimum of three years of relevant work experience, preferably in an oncology healthcare setting.
Knowledge and experience working in mediical settings and interacting collaboratively with medical teams.
Knowledge of community resources, medical terminology and a working knowledge of HIPPA.
Compassionate and patient.
Strong problem solving, relationship building and advocacy skills.
Excellent verbal, written and interpersonal communication skills and the demonstrated ability to work with diverse individuals and groups.
Ability to respond to changing circumstances and priorities in a positive, focused, and timely manner. Proactively addresses issues as they arise.
Remains composed and professional under stress and takes responsibility for delivering on personal and organizational commitments.
Proficient knowledge in Microsoft office and other software applications, Internet-based applications and computer equipment
Willingness to travel and work beyond normal business hours as required to achieve agreed upon objectives.
