Job Description
Job Summary:
The Physical Therapist works with and reports to the Home Health Coordinator.  The Physical Therapist is a licensed/registered professional who has completed an accredited program of education.  The Physical Therapist's primary function is to plan and administer medically prescribed or referred physical therapy treatment programs for patients to restore function, relieve pain, and prevent disability following disease, injury, or loss of body part.  He/she also administers and directs administration of physical agents utilized in physical medicine and rehabilitation as prescribed by a referring physician.  All records of patient care are maintained accurately. Uses and Discloses patient protected health information: 1) Only as it applies to job functions, 2) in amounts minimally necessary for intended purpose, 3) in a confidential manner. Essential Duties:1.   Administers physical therapy treatments per physician order.2.   Assists the Home Care Coordinator with the development, review, and revision of standards for home physical therapy services.3.   Participates in QI activities of agency.4.   Communicates assertively and openly about patients' well-being, treatment, and comprehensive care with physician and home care staff.5.   Maintains records pertinent to patient treatment.6.   Appropriately supervises the Physical Therapy Assistant.7.   Trains and educates staff personnel, students, community, families, etc.8.   Maintains standards as set down by the American Physical Therapy Association regarding treatment procedure and professional ethics.9.   Interprets diagnosis and prescribing treatment to achieve maximum benefit and safety.10.  Performs an initial evaluation of the patient for the purpose of assessment and the gathering of sufficient information to determine long- and short-term goals and to plan the physical therapy program.11.  Documents the on-going progress of the patient on a timely and outlined format as described in the agency policy and procedure manual.12.  Consults with the patient's referring physician and the Home Health Coordinator (if necessary).13.  Re-evaluates the patient's status and the patient's physical therapy program so that the program can be modified or discontinued.14.  Establishes home programs specific to the patient's needs and verifying that the patient understands the rationale and is able to demonstrate the program correctly.15.  Determines, evaluates, and modifies equipment, prosthetic, and orthotic needs.16.  Schedules patient visits with respect to consistency, flexibility, efficiency, and departmental ability/availability.17.  Administers correct treatments.18.  Maintains safety of property and patient under treatment.



Job Requirements
Education:
  • Completion of Bachelor's Degree in Physical Therapy from a school accredited by the American Physical Therapy Association.
Experience:
  • Two years of experience as a Registered Physical Therapist preferred.
  • Experience as a home care Physical Therapist preferred.
License/Certification: 
  • Must be registered in the State of Nebraska as a Registered Physical Therapist.
  • Must have a valid Nebraska Drivers' License and a clean driving record.
