We are seeking an experienced Service Coordinator/Care Manager with a long-term care background to join us in a rewarding role. As our newest Service Coordinator/Care Manager, you would visit patients in their homes or other community settings, acting as their advocate and helping them to stay healthy and independent in their current living situation. Job Requirements

Primary responsibilities of the Care Coordinator include:

  • Visiting patients in their homes, assisted living facilities and other community-based settings
  • Delivering a holistic approach to coordinated care based on member needs
  • Identifying early risk factors and conducting ongoing assessments
  • Creating care plans and focusing on disease management
  • Functioning as an advocate for members and decreasing unnecessary hospitalizations
  • Collaborating with the member's Physician to deliver and coordinate necessary services
  • Building relationships with members and their families; assisting them with proper health choices
  • Ensuring cases are documented in a timely manner
  • Participating in team meetings

The requirements for this role are as follows:

  • A minimum of 1 year of clinical experience
  • 2 years of Case Management experience
  • Background in long-term care (could include: home health, hospice, public health, assisted living and community-based nursing)
  • Computer proficiency
  • Must reside within the service area for DSL or Cable

Additional assets include:

  • Bilingual Spanish or Native American and English speaking skills
  • Medicare/Medicaid experience
  • Discharge planning experience
  • A background working in pediatrics special needs
  • A background in behavioral health
  • Experience with electronic charting
