Description The Diabetes Care Management Program for adolescents, part of VNSNY's Children and Family Services, offers intensive nursing assistance, guidance and support to low-income, first-time mothers in the Bronx. This national initiative has a history of documented success at improving pregnancy outcomes and enhancing the long-term health and well-being of young, impoverished mothers and their children.


As Manager, you will be responsible for the management and development of the new Diabetes Care Management Program for adolescents. You will manage staff and oversee/participate in the provision of diabetes education and care management through a collaborative process with clients, based on assessment, goal planning/monitoring, coaching, data management, and connecting to community resources consistent with the program's policies and protocols. You will evaluate data and provide consultations related to clinical outcomes of patients.

Location: Upper Manhattan / Bronx
Requirements This position requires a Bachelor's or Master's degree in Nursing or Nutrition, a valid driver's license and at least five years of clinical experience which includes a minimum of 3 years of diabetes experience. In addition, license and current registration to practice as a Registered Professional Nurse, Registered Dietician, a licensed Clinical Social Worker, or other discipline (as per Certified Diabetes Educator requirements) in New York State is required. On path for certification or significant diabetes experience (pediatrics preferred) and interest in pursuing certification required. Current certification as a Certified Diabetes Educator by the National Certification Board for Diabetes Educators preferred. Minimum of two years managerial experience and clinical expertise in behavioral health and adolescent/pediatrics and home visiting experience are all preferred.
