A weekend rotation is possible every 4th-5th weekendJob Title:Clinical Pharmacy Specialist Department / Area:PharmacySUMMARYCoordinates educational programs and activities designed to enhance clinical drug knowledge of staff pharmacists and increase their participation in drug monitoring, pharmacokinetics and cost containment activities. Provides pharmaceutical care to patients in assigned area of practice, implement cost containment programs, provide, drug information, education and clinical services which enhance the effectiveness of drug therapy and minimize patient risk. QUALIFICATIONS EDUCATION and / or EXPERIENCE, LICENSURE AND CERTIFICATIONBachelor’s Degree (5 year) in Pharmacy. Licensed as a registered pharmacist by the State of Michigan.Two years hospital pharmacy experience or a ASHP Pharmacy Practice Please see preferences for this position under the requirements tab.Effective teaching abilities. Competent pharmacokinetic skills. Excellent written, verbal communication and interpersonal skills. Time management and organizational skills. Self directed. Independently mobile. Ability to make quality, independent decisions. Analytical and strong problem solving skills. Ability to work effectively and efficiently under tight deadlines, high volumes and multiple interruptions.PREFERRED KNOWLEDGE / SKILLS / ABILITIES / EDUCATION / EXPERIENCEASHP accredited residency in Pharmacy Practice. Board Certified Pharmacotherapy Specialist. Doctor of Pharmacy Degree, CPR Certified. ACLS preferred. Clinical practice experience preferred.PHYSICAL DEMANDS / WORK ENVIRONMENTExposure to drugs and other agents in the Pharmacy area. Possible exposure to a variety of contagious diseases. Exposure to a variety of attitudes, personalities and health related reactions from patients, families and visitors...ESSENTIAL FUNCTIONS include the following. Other duties may be assigned.Evaluates patients and orders, and takes a lead role in the ordering and interpretation of drug serum levels, using pharmacokinetic principles. Provides physicians with dosage regimen recommendations to enhance therapeutic outcomes reduce patients risk and reduce the cost of drug therapy. Provides drug information and educational services to hospital health care professionals, patients and families and assists staff pharmacists in responding to drug information requests. Serves as an information resource and clinical practice role model for staff pharmacists. Coordinates and implements departmental drug utilization review efforts and other process improvement activities necessary for meeting JCAHO requirements. Facilitates Pharmacy and Therapeutics Committee activities in area of specialty and participates on other committees / task forces as member, consultant and/or chair. Coordinates the development, editing and dissemination of departmental publications for physicians and hospital staff. Performs other duties and responsibilities as assigned. Ensures that processes and services are continuously monitored for quality, cost effectiveness, and efficiency. Engages in process and quality improvement activities. Makes and implements recommendations to improve operational efficiency and to implement new services for areas of responsibility.Ensures that services are provided in accordance with state and Federal regulations, organizational policy, and accreditation / compliance requirements. Performs such individual assignments as management may direct. Establishes and maintains effective working relationships within the organization. Provides information to senior levels of management on the implication of policies and procedures being formulated and recommends specific action. Engages in continuous study of the entire professional field, including best practices, to maintain the professional competence, knowledge, and skills necessary for the satisfactory performance of all assigned responsibilities. Completes required continuous training and education, including department specific requirements. Ensures that required documentation for activities within areas of responsibility is performed in an accurate and timely manner.Ensures that patients, their families, visitors, customers, and employees are treated with kindness and respect. Demonstrates teamwork and integrity in all work-related activities.
