Responsible to the Manager for providing coordination of efficient, cost effective quality care along the continuum of care within identified patient care setting. Case manages the direction and support for the achievement and monitoring of an identified patient care outcome. Collaborates with psychosocial case manager any cases which demonstrate actual of potential psychosocial/discharge planning needs and refers cases to appropriate community agencies. Collaborates with multidisciplinary team when available. EDUCATION/CERTIFICATION/FORMAL TRAINING: A Bachelor degree of an accredited school of Respiratory Therapy, Pharmacy, Physical Therapy, or Nursing is required. A Masters degree is preferred (prior to January 1, 1997, an A.D.N. has been granfathered into this position). Must hold current Indiana license.
