The nurse care manager is accountable for managing an assigned patient population along the continuum of care from admission through discharge. The nurse care manager facilitates the delivery of cost effective, high quality care with emphasis on education, customer satisfaction and quality outcomes. The nurse care manager is responsible for assisting in maintaining a financially stable unit, promoting his/her own development with an emphasis on evidence based practice and education supported by Practice Governance framework.
The responsibilities of the nurse care manager include: assessing the medical necessity of the admission, continued stay and discharge plan according to established criteria such as InterQual, Milliman USA, and hospital approved disease/procedure clinical pathways; emphasis on creating a positive customer experience (including patient, family, physician and staff); maintaining up to date knowledge of nursing practice, teaching techniques and professional nursing practice; use of nursing judgment and adherence to the code of professional conduct of the Commission for Case Manager Certification (CCMC).
