Job Description
The Case Manager serves as the interdisciplinary team leader to provide management of a patient's hospitalization from preadmission to post discharge and is an effective communicator and manager of information in coordinating health care decisions. Primarily, the Case Manager oversees the effective coordination of services in the areas of: insurance relations, fiscal management, admission and discharge coordination, case conferencing and communication relative to the overall treatment plan. As the interdisciplinary team coordinator, the Case Manager facilitates communication amongst the team members regarding delivery of services to patients and families, performs ongoing utilization review, acts as a liaison to the payor, assures that cost effective treatment is provided by the team, facilitates timely communication regarding the patient's care, establishes and implements the discharge plan. The Case Manager assures that regulations regarding patient's rights are fulfilled, and addresses the patient's psychosocial status and support systems issues. The Case Manager may work in either the inpatient or outpatient setting.
POSITION REQUIREMENTS
License or Certification:
Current state licensure/certification, if applicable
Total Education, Vocational Training and Experience:
Bachelors or masters degree in social work, rehabilitation or related allied health field or a degree in nursing (BSN preferred) with appropriate valid state license/certifications (CRRN, CIRS, CRC or CCM preferred)
Machines, Equipment Used:
General office equipment such as telephone, copy machine, fax machine, calculator, computer.
Physical Requirements:
Good visual acuity and ability to communicate
Ability to lift, push, pull, and retrieve approximately 10 percent of the time.
Ability to safely lift objects weighing up to 30 pounds.
Ability to demonstrate safe retrieval skills from above head to floor level with objects up to ten pounds.
Skills and Abilities:
Ability to speak, read, write and comprehend English.
Ability to coordinate, analyze, observe, make decisions, and meet deadlines in a detail-oriented manner
Ability to communicate effectively in both writing and verbal arenas as well as via email.
Ability to work independently without supervision.
Environmental Conditions:
Indoor, temperature controlled, smoke-free environment. Occasional outdoor exposure
Exposure or potential exposure to blood and body fluids may be required.
Handicapped accessibility.
May work under stressful circumstances at times.
Proficiency or Productivity Standards:
May be required to work weekdays and/or weekends, evenings and or night shifts if needed to meet deadlines.
May be required to work on religious and/or legal holidays on scheduled days/shifts.
Will be required to work as necessary during disaster situations, i.e. before, during or after a disaster.
May be required to stay after workday to assist after a disaster situation until relief arrives.
Regular attendance and reporting on time to work is a requirement of position.
