New Mexico announces a new statewide, coordinated long-term care program in which Evercare will be one of two health plans serving close to 40,000 Medicaid recipients. Evercare is a premier clinical care management organization, and collaborates with federal and state leaders to create new programs to serve this population. We are seeking an experienced RN with a long-term care background to join us in a rewarding role in the Farmington location. As our newest Clinical Care Manager, you would visit patients in their homes or other community settings, acting as their advocate and helping them to stay healthy and independent in their current living situation. This position is available in the Grants/Gallup area.
 
Primary responsibilities of the Clinical Care Manager, RN include:

  • Visiting patients in their homes, assisted living facilities and other community-based settings
  • Delivering a holistic approach to coordinated care based on member needs
  • Identifying early risk factors and conducting ongoing assessments
  • Creating care plans and focusing on disease management
  • Functioning as an advocate for members and decreasing unnecessary hospitalizations
  • Collaborating with the member's Physician to deliver and coordinate necessary services
  • Building relationships with members and their families; assisting them with proper health choices
  • Ensuring cases are documented in a timely manner
  • Participating in team meetings



Job Requirements

The requirements for this role are as follows:
  • An RN with a current and unrestricted license in their state of residence
  • Substantial clinical experience
  • A background in long-term care (could include: home health, hospice, public health, assisted living and community-based nursing)
  • Computer proficiency
  • Within the service area for DSL or Cable
Additional assets include:
  • Case management experience
  • Bilingual Spanish or Native American and English speaking skills
  • Medicare/Medicaid experience
  • A background working in pediatrics special needs
  • Experience working with patients with compromised respiratory function (using a ventilator)
  • A background in behavioral health
  • Experience with electronic charting
