Job Description
Job Summary: Responsible for providing comprehensive nutrition screening,assessment and care planning for St.Clare Medical Outreach patients Provides client and staff education. Participates on hospital committees and council as directed and participates in community events as appropriate. Essential Duties: - Develops client relationships and client satisfaction for nutrition assessments and care planning of patients
- Uses culturally competent skills to assess individual patient needs
- Documents in the clinical record a comprehensive assessment of patients needs, barriers, interventions, resources and plan of care.
- Ensures service is customer focused timely, informative and individualized
- Directs dietary care of patients including screening, assessment, intervention and planning
- Accountable for service quality
- Promotes and supports a culturally diverse population
- Develops and implements nutritional programs
- Evaluates programs and reports results
- Confers with medical staff and other health professionals to coordinate medical and nutritional needs
- Educates medical staff and counsels patients of families on proper nutritional care
- Stays current with nutrition-related trends, research, policy, and certifications
Job Requirements
Education: - BS in Food and Nutrition, Masters preferred
- Bilingual in Spanish and English
- One year experience as a Clinical Dietician preferred who has worked with immigrant and uninsured population.
License/Certification: - Registered Dietician certification.
- Licenced to practice in this state.
