RN, Case Manager, RN home care

Excellent opportunity for RN to independently manage an established home care caseload. Must have excellent clinical assessment skills, the ability to lead a multidisciplinary team of professionals, create and coordinate specific physician approved plan of care, and possess sound organization skills. The selected candidate will be provided an extensive orientation which address clinical care in the home care setting, management, communication and utilization of lab top topics
