Description
Hometown Home Health & Hospice has an opening for  an experienced Physical Therapist to service clients in Mid-Michigan.  We have experienced a great deal of growth and expansion in the past year and need a full time physical therapist.

The Physical Therapist is an intricate part of the multi-disciplinary team who evaluates and treats a variety of patient's physical therapy needs. The ideal candidate will have excellent patient care skills, customer service skills, and top notch written and verbal communication skills.

The Home Health Physical Therapist:

  • Provides a variety of physical therapy services to clients in their place of residence in accordance with physician's authorized plan of care.
  • Assists the physician in assessing the patient's level of functioning and the development (and revision, as needed) of the plan of care by applying appropriate tests for physical therapy evaluation.
  • Demonstrates broad base knowledge of accepted disease/post surgical intervention specific physical therapy evaluation, rehabilitative exercise, treatments, and teaching techniques.
  • Assures coordination and continuity of plan of care by maintaining lines of  communication with other health care professionals assigned to the same patient.
  • Attends patient care conferences, and initiates patient care conferences for complex and/or multidisciplinary patients whenever needed to insure good coordination of care.
  • Evaluates and regularly reevaluates the physical therapy needs of the patient; initiates, develops, implements and makes necessary revisions to the patient's plan of care.
  • Demonstrate the knowledge, skills, and compassion necessary to provide care to the geriatric population.
  • Able to assess data reflective of a patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs.
  • Additional administrative, management and leadership duties as required.

 

 

 

Requirements Requirements are as follows:

  • Must have positive attitude and excellent bedside mannor.
  • License to practice as a physical therapist in the State of Michigan.
  • Current Michigan Operator's License.
  • Has reliable transportation and willing to drive in all kinds of weather.
  • Skills necessary to be successful using paperless documentation system.

 

Corporate office is located in Lapeer, MI and position is located in Midland, MI.
