Description

The American Cancer Society seeks an experienced Patient Resource Navigator. This position will be based in Charlotte,NC.

This is a three year grant funded position with high potential for becoming a regular position. Full benefits offered. Position is located at Carolinas Medical Center Blumenthal Cancer Center. Oncology social worker or health care professional desired. Cancer Survivors are also encouraged to apply.**

Job Description
Works with at-risk hospital patients and assist them in accessing the hospital's cancer care delivery system and ACS resources. This will include direct interaction with patients and their families in person, over the phone, and by email. Responsible for assessing the needs of the hospital's cancer population, providing programming for these needs and developing the infrastructure (i.e., volunteer program).
Collaborates with hospital cancer center staff to provide the Society resources and support as well as hospital services to appropriate patients. Works collaboratively with and takes day-to-day instruction from Administrator at Cancer Center. Implements processes to assess needs, prevent pre-arrival barriers, and access proper resources for cancer patients, i.e., transportation, financial concerns, etc.
Meets with patients to help and direct them. Monitors on-going needs of patients to ensure efficient and effective access to services.
Responds to inquiries and interprets policies and programs to the public.
Evaluates, develops, markets and implements collaborative initiatives for patient population to promote supportive care programming.
Develops core of hospital volunteers to deliver programming to constituents. This will include assistance to patients in accessing appointments, navigating different locations, etc.
Builds relationships with community network and screening programs to increase access to patient care and the Society.
Creates and supports interface between appropriate Society and Hospital communities and resources to maximize benefit to both.
Makes business based decisions; maintains accurate files, submits timely and accurate reports; meets timelines, follows through; apprises supervisor of status; provides viable & innovative solutions to problems; attends meetings/trainings, adheres to approved policy/procedure, represents the organization in a professional manner, makes effective presentations. Proactively and assertively addresses issues as they arise. Performs other duties as assigned.

 

 

Requirements

MINIMUM QUALIFICATIONS:
Bachelors degree in social work or health sciences or equivalent combination of education and experience.
Three years related work experience. Valid drivers license and reliable transportation required.

OTHER DESIRABLE QUALIFICATIONS:
Experience in an oncology outpatient setting.
Experience working with culturally diverse populations.
Must possess strong working knowledge of medical systems.
Able to establish strong working relationships within medical systems.
