Case Manager (Per Diem)

Under the supervision of the Director, the Case Manager will perform case management and utilization review functions; performs discharge planning; and collaborates with the patient care team for the purpose of optimizing patient care and resource usage.

Responsibilities also include:

• Facilitate the interdisciplinary plan of care with a focus on evaluating the appropriateness of clinical care, medical necessity, admission status, level of care, and resource management.

• Coordinate activities that promote quality outcomes and patient throughput while supporting a balance of optimal care and appropriate resource utilization.

• Identify potential barriers to patient throughput and quality outcomes.

• Facilitate appropriate discharge plans in collaboration with social worker. 

• All duties are performed consistent with the philosophy in a manner consistent with the mission and goals of the organization.

• Current RN license

• Graduate of an accredited professional school of nursing or social work

• Bachelor’s degree preferred

• 3 years nursing experience in acute care setting; Case Management experience required

• Clinical knowledge, experience in utilization review, discharge planning and payor systems; ability to communicate effectively with members of the healthcare team

• Certification in Case Management, Nursing, or Utilization Review preferred
