Description
Indiana Professional Management Group (IPMG) provides consolidated case management services for Hoosiers with developmental disabilities that are on the Medicaid waiver programs.
We have an opening for a full-time blended role as a:
Case Manager Supervisor (PT)/Case Manager (PT)
Covering: Dubois, Spencer, Orange, Crawford, Perry, Washington, and Harrison
counties located in Indiana.
Case Manager Responsibilities (Personal consumer caseload to 23 consumers): Case Managers communicate with guardians, service providers and state agencies, participate in the creation and implementation of the Individual Support Plan through the process of Person Centered Planning, link individuals to community services, monitor progress toward outcomes, implement procedures for quick response to crisis, and maintain appropriate records. Case Managers work out of their homes and set flexible schedules based upon the need of the consumer. Initial and ongoing support and training are provided. Case Managers generally travel within a limited geographical area within the state.
Case Manager Supervisor Responsibilities: Provide support and assistance to Case Managers for quick response to consumers in crisis including coordinating necessary actions, authorizing short-term services, and involving providers or other entities as need to resolve the crisis. Case Manager Supervisors actively participate in management team meetings. Maintain IPMG performance standards by ensuring that assigned team members are in compliance of with assigned actions and data management requirements. Case Manager Supervisor’s work out of their homes and set flexible schedules based upon the need of the consumer and generally travel within a limited geographical area within the state.
Requirements
Qualifications include a Bachelor’s degree in a human services related field or a Bachelor’s degree in any field and a minimum of two (2) years direct experience working with individuals with developmental disabilities managing DD waivers within Indiana, and two (2) years supervision experience.
The qualified candidate’s skills will include; having strong planning/organization, attention to detail, the ability to manage multiple tasks technology, interpersonal (oral & written) communication, the ability to oversee a virtual team, problem-solving, conflict management, bias for action, and has empathy for persons with developmental disabilities.
