Location: Butler and Hamilton County

• Ability to work on call at various locations  
• Generate service ticket from on-line form   
• Notify care coordinator of appointment via telephone, create chart, enter intake information in Unicare database and request DAF, IEP, psychiatric Hx and all pertinent information from care coordinator. 
• Refill client Rx’s per existing refill process.    
• Schedule client appointments.    
• Manage day-to-day program processes.    
• Review client chart to ensure laboratory results have been ordered and received, permission to administer medication forms are current, consent to treat current, AIMS form current, review, update and print out medication list, review all billing information and update when necessary and ensure chart compliance per MHB and ODMH regulations and guidelines. Transcribe stat dictation end of clinic day.    
• Enter completed service tickets into Unicare for billing.

• Individual must be a Licensed Practical Nurse, with a Pharmacology Card, from an accredited school.
• Must be licensed by the State of Ohio
• 1-2 years nursing experience. Experience in psychiatric nursing is desirable.
• Candidate must have a valid driver’s license, a good driving record and proof of auto insurance.
