Requirements

Certified Occupational Therapist Assistant / COTA / OTA

Speech Language Pathologist / SLP

  

  

* Active Indiana COTA or SLP license.

* Current CPR certification.

* Physical Exam and PPD or Chest X-Ray within the last year.

* Three professional therapy references.

* Professional appearance and strong work ethic.

* Must pass a company-paid criminal background check and drug screening.
