Description:
We are currently seeking a full-time experienced Medical Social Worker for our Sandusky, Ohio home health agency. This position requires a Master’s Degree in Social Work, plus Licensed Social Work (LSW) certification by the Ohio Counselor, Social Worker, Marriage and Family Board.
The Medical Social Worker assists and counsels home health patients and families with health related financial, social and emotional concerns according to the physician’s orders.

Requirements
Requirements:
1. Meets state and federal requirements for this position, including a Master’s Degree in Social Work, plus Licensed Social Work (LSW) certification by the Ohio Counselor, Social Worker, Marriage and Family Board
2. Minimum of one (1) year of experience in social work in a health care setting.
3. Current CPR certification.
4. Personal transportation, valid drivers license and automobile insurance coverage.
