Home Health Branch Supervisor - Director of Nursing - Nurse Manager

The VNA of Florida has a current need for Branch Supervisor to oversee day-to-day operations of our Spring Hill, FL office.  This position is responsible for managing Hernando County.

 

SUMMARY

To ensure delivery of quality care to patients through supervision/oversight of clinical field staff and coordination of agency services. To assure compliance with agency policies and procedures, current physician's orders, appropriate interventions and reports to the physician, and complete and accurate documentation in the clinical record.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES

 

  • Ensures quality and safe delivery of home health care services by supervising, training, and directing field and administrative staff.
  • Training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.
  • Ensures that staff demonstrates competency in necessary clinical skills.
  • Ensures that admitting staff are competent in conducting comprehensive assessments, taking appropriate histories, and evaluating home situations to determine if cases are appropriate for home care.
  • Ensures that OASIS assessments are done accurately, according to instructions in Chapter 8 of the OASIS Implementation Manual. 
  • Ensures that staff is competent in understanding and applying guidelines. Presents and explains the Medicare Advance Beneficiary Notice form to Medicare patients when necessary. .
  • Provides consultation to team staff in establishing (based on the physician's orders) a plan of care, clinical pathway, goals, interventions and schedule of services and disciplines to meet the patient's needs. Assists staff in determining appropriate Primary Diagnosis and ICD-9 codes.
  • Reviews completed documentation (including physician's orders, OASIS and Initial Assessment, Plan of Care, and visit schedule) to ensure accuracy and continuity.
  • Is responsible for assuring that any Physician's Orders (POC or Verbal Orders) are reduced to writing to be signed by the physician, and assuring that those physicians orders are followed.
  • Assures that the patient's condition is correctly assessed and that team staff report significant changes to the physician, and other team members.  Assures that written progress reports are made to the physicians as needed.
  • Is responsible for accuracy of team census, and that all active patients are seen and cared for as ordered.
  • Ensures that Case Managers coordinate and direct the course of care, by all disciplines, for all  patients in case load.
  • Provides guidance and direction to the Licensed Practical Nurse and/or float nurses who are assisting with the case load, should the Case Manager be unavailable. Consults with physician and interacts in establishing or altering plans of care when the staff is unable to do so.
  • Assist staff to efficiently manage the type and amount of care provided to meet the goals with the resources provided. 
  • Ensures that team staff complies with agency regulations regarding documentation and maintains clinical record for each patient.  Ensures that clinical notes/contacts/physician's orders are turned in/communicated the day of service. 
  • Chairs and facilitates Team Conferences, directing team members in discussions for specific problem solving measures for patients in case load, and checks to assure that conferences are documented and recommendations are carried out.  
  • Coordinates staff PDOs, weekend, holiday, and On-Call coverage to assure adequate staffing at all times. Fills in as field RN on an emergency basis.
  • Ensures that Full- time staff maintains a productivity standard of at least 33 units per week (66 per pay period) before utilizing per visit staff. Keeps D.O.N. apprised of staffing levels and needs.
  • Participates in Performance Improvement activities.
  • Maintains client/patient confidentiality in accordance with the Florida law and the Health Insurance Portability & Accountability Act (HIPAA).
  • Fulfills additional performance responsibilities as assigned and as necessary.
Requirements

SUPERVISORY REQUIREMENTS - Minimum 2 years supervisory/management experience.

Directly supervises the clinical field staff.  Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws.  

 

QUALIFICATIONS

  • Passing score on Agency's Core Nursing Test
  • Minimum BASIC computer skills, ADVANCED computer skills preferred.
  • Passing score on Agency's Medicare Test
  • Available for on-call duty night and weekends and must be accessible by phone
  • Active Florida Nursing License for occasional field visits
  • Must have use of automobile
  • Knowledgeable in current standards of practice.
  • Understands the home health coverage guidelines for Medicare/Insurance and determines whether the care required meets those guidelines.
  • Understands the various payment options that are available

 

EXPERIENCE

  • Minimum one year of hospital experience in the field of Med/Surg nursing
  • Minimum 3 years Home Health experience
  • Minimum 2 years supervisory, management, or DON experience

 

EDUCATION

Graduate of an approved school of nursing and is currently licensed in Florida.
