Certified Physical Therapy Assistant/PT /Facility /Home Care
NurseCore, an industry leader in Medical Staffing and Home Health Care, has exciting opportunities for a Certified Physical Therapy Assistant for Home Care Clients and Facilities. We are offering long term contracts, per diem and temp-to-perm opportunities for Home Care and Private Duty medical office professionals 24 hours a day, seven days a week.
As a Per Diem Therapist you will provide treatment as prescribed by an authorized practitioner in accordance with our agency policies and procedures, you will develop and update Plans of Care for clients, including treatment goals and will perform client visits in the home and/or facility as directed by the Plan of Care.
Per Diem Therapists will report directly to the Agency's Clinical Director.

Requirements
- Current State Certification
- Minimum of one (1) year experience in a supervised setting within the last three (3) years
