Case Manager - Social Work - Social Services
Case Manager – Placement Services/TEEN MOM (Foster Care)
Perform case work services for children and families in regular, specialized or home-of-relative foster care programs with an emphasis on meeting the clients’ environmental and psychosocial needs, and implementing a permanent living plan for each child; build and/or manage a caseload of foster care providers. Previous experience working with TEEN MOM programs required.
Essential Functions of Case Manager/Social Work Position – conduct monthly home visits or more frequently as the program requires; provide case work services to children in placement, their parents and/or guardians; arrange and supervise visits between children and their birth parents, coordinate collateral contacts and services as needed; work cooperatively with other agencies and service providers in the performance of case work duties; prepare, maintain and provide documents and reports of cases and appear and/or testify in court as necessary; complete data entry into client management systems; consult and review cases with supervisor and follow course of action; participate in ongoing training and meet all agency and DCFS requirements; work effectively with multi-disciplinary team to do what is in the best interest of the child/family.

Requirements
Case Manager - Social Work - Qualifications
- Bachelor’s degree in Social Work, Human Service, Psychology or similar degree from an accredited university
- DCFS required examinations and certifications including the DCFS Child Welfare Licensure
- Previous experience working with TEEN MOM programs required.
- Maintenance of required driving standards, plus insured automobile that meets standards of safety and driving required
- 1-3 years experience preferred
- Flexibility and willingness to work evening and/or weekend hours, if needed to carry out job responsibilities
