Description
Patient Care Coordinator
Major Hospital/Medical Center located in New York City is currently seeking a qualified Patient Care Coordinator for their expanding department.
In this position, you will coordinate the admissions, transfers and discharges of patients, ensuring they are effectively and efficiently provided comprehensive, high-quality care.
Responsiblities include assessing patients of all ages; developing and coordinating interdisciplinary care plans and modifying them as needed; monitoring resource utilization as well as nursing staff performance; helping develop discharge plans; and evaluating outcomes. You will also be expected to provide direct care to patients and serve as a resource to their families.
Requirements
The ideal candidate should have a BSN degree and 5+ years experience in critical care, cardiac cath and strong patient care coordination. NYS RN IS A MUST.
The starting salary for this position is up to 90K Base with excellent benefits and visibility.
