Our respected medical facility in the Nashville, TN area is looking for an experienced Clinical Pharmacist Coordinator for our Pharmacy.
LEADERSHIP
*Clinical leadership for the Department of Pharmacy
*Coordinates and schedules regular clinical staff meetings
*Leads clinical development projects (conceptualize, design, implement, evaluate, and report). *Coordinates implementation of clinical projects to meet corporate, division, and hospital goals.
*Leads, analyzes, and appraises clinical pharmacy services provided to patients, physicians and ancillary departments including but not limited to pharmaceutical care, consultative services and drug information services in specialty areas of Women’s Services, Internal Medicine, Surgical, Medical & *Neonatal ICU and Psychiatry.
*Maintains an active role as a member of the Pharmacy and Therapeutics Committee, Forms Committee, Investigational Human Research Committee, and other multidisciplinary committees at the discretion of director.
*Evaluates clinical skills of pharmacists and provides performance review input
*Leads the resolution of medication related problems in a clinical practice area.
*Coordinates implementation of clinical initiatives with the Network Clinical Coordinator and site manager to develop a cohesive environment with consistent direction and goals at the site level.
*Oversees safe and appropriate pharmaceutical care for the patients by developing and utilizing evidence-based and best-practice protocols. Develops and implements policies and procedures. *Promotes and supports safe medication management.
*Develops, implements, and evaluates treatment guidelines that address cost-effective and appropriate drug therapy.
COMMUNICATION
*Has excellent communication and interpersonal skills.
*Applies knowledge of marketing techniques to promote and facilitate adherence to established guidelines and formulary status.
*Shares responsibility for departmental publications, educational programs and pharmacy initiatives to promote pharmacy as a respected source of drug therapy information and disease management resource.
*Participates in interdisciplinary subcommittees to enhance patient care.
*Communicates directly with physicians, nurses, and other health care providers to relate information regarding patient drug therapy.
*Communicates and documents drug information given to patients and/or patient representatives.
QUALITY
*Oversees, monitors, and evaluates the intervention program data
*Evaluates, reports, and consults with physicians and nursing staff regarding patient adverse reactions, errors and complaints.
*Assists with quality improvement initiatives and ordering of pharmaceuticals during times of drug shortage.
*Oversees the development and maintenance processes of formulary review, medication use evaluations, therapeutic drug protocols, and drug information.
*Initiates, analyzes, and reviews monthly and annual reports to determine compliance with corporate, division, and departmental clinical initiatives and value propositions. Works with staff to achieve corporate, division, and department clinical goals.
TEACHING
*Educator of staff, nursing, medical staff, patient and family on medication issues
*Maintains faculty appointment with the UT, Mercer, Lipscomb, and Belmont. Precepts and coordinates the precepting of pharmacy students; serves as a role model to pharmacy students; and acts as a project advisor to the College in areas of expertise or as assigned by the Director
*Provides, coordinates, and evaluates staff development opportunities for practicing pharmacists and technicians.
CONSULT
*Provides on-call support and back fill support for the pharmacy within direct clinical pharmacy services, consultative services and drug information services to patients and health care providers directly and through committee work
*Provides back fill support for the pharmacy within distribution services by accurately, efficiently and appropriately interpreting physicians’ drug orders, entering orders into the department’s computer system, and dispensing the appropriate medications utilizing the accepted departmental policies. *Identifies, resolves, and prevents potential and actual drug-related problems by obtaining and evaluating medical data including patient history, laboratory data, drug interactions, allergies, contraindications, adverse drug reactions.
SELF
*Maintains familiarity with all departmental programs and works as an integral part of the pharmacy team.
*Maintains and updates personal drug knowledge database to ensure accurate provision of pharmaceutical care; maintains advanced practice base through the avenues of publications, presentations, educational boards, professional consultation services, association leadership, and certification.
*Conducts personal practice in accordance with state and federal regulations and statutes, hospital policies and procedures, and pharmacy mission statement.
Requirements
Required Qualifications
Education:
BS in Pharmacy (advance degree preferred) Residency training
is preferred. Current licensure in Tennessee, or eligibility for
licensure is required.
Experience:
5 years of hospital experience and demonstrated clinical skills in drug information, pharmacology, pathophysiology, advanced pharmacotherapeutics, pharmacokinetics, pharmacoeconomics and advanced clinical pharmacy practice is required. ; Managerial/supervisor experience preferred.
Skills:
Proficiency in Microsoft Office, and project management skills is required.
