My client is a first class western suburban hospital (400+beds) which has been recognized as one of the top 5% of hospitals nationwide for their dedication to clinical excellence. They are financially stable, have recently adopted the Planetree model of care and are in the final stages of construction on a brand new hospital.
Requirements
The case manager wil be responsible for coordinating and facilitating care of selected patients throughout the inpatient episode as a key member of the Case Management Team. Includes activities related to resource utilization applying utilization review processes. Facilitates the coordination of efforts to decrease length of stay and utilization of resources while improving quality of care.
Education: Graduate of an accredited school of nursing.
Experience: A minimum of 4-5 years acute care clinical experience, preferably in Medical Surgical Nursing. A minimum of 2 years recent Utilization Management experience with working understanding of payor requirements, including admission criteria preferred.
