Pulaski Health & Rehab Center is seeking a Director of Discharge Planning for a 90-bed skilled nursing center.
The Director of Discharge Planning will direct and supervise the social service department within the framework of MFA’s standards for best practices and in accordance with state and federal regulations. Coordinates and expedites patient discharge planning initiatives from the day of admission through the day of discharge and provides all other comprehensive medically related social services identified and needed by the patients of the Center.
