Lutheran Family Services of Virginia (LFSVA) is a nonprofit agency that provides case management, community-based services, therapeutic mentoring, and developmental services for autism at six locations in Virginia.

SERVICES WORKERS (Richmond) - Therapeutic Foster Care & In-Home Services

In this dynamic position, we are seeking a candidate that has the experience and ability to work in both our therapeutic foster care program and our in-home services program.  Service Workers provide on-going services to children and families such as home visits, case management, home studies, therapeutic intervention, development of treatment plans, linking families with community resources, family support, and serve as a team leader.

During the course of your work you will:

 

  • Perform tasks related to admission or placement such as meeting with families to provide information, discuss fees and; complete eligibility determination and assessments.

  • Create individual service plans

  • Interface with referring agencies & provide required documentation

  • Coordinate with various referring agencies & agency staff

  • Act as team leader while coordinating with the service planning team to coordinate meetings, assign tasks and provide guidance and direction.

  • Identify & link families with appropriate services & other community resources in a timely fashion.

  • Depending on educational background, experience and ability, perform individual and family counseling as appropriate.

  • Be responsible for case documentation.  Maintain accurate, complete and up to date files on all youth and parents assigned.

  • Demonstrate knowledge and compliance with state licensing, Medicaid and other accreditation standards.

  • Complete periodic progress reviews and documents as required.

  • Participate in crisis intervention and provide support as required, including providing intervention to prevent disruption of services. 

  • Participate in on-call rotation as scheduled.

 

While performing the above tasks, you must also ensure that services are provided in accordance with the individual service plan and documented as required by agency procedures and licensing/funding agency standards.

Requirements
  • Master's degree in Social Work or other Human Services Field preferred & min. 1 year clinical experience working with families and children OR

  • Bachelor's degree in Social Work or other Human Services Field (psychology sociology, counseling) & minimum of 2 years clinical experience working with families and children

  • Knowledge of and ability to comply with state licensing, Medicaid and other accreditation standards.

  • Good Verbal and Written Communication Skills

  • Good Interpersonal Skills and ability to work effectively with people from diverse backgrounds

  • Ability to work a flexible work schedule including some weekend & evening work

  • Good Organization & Administrative Skills

  • Basic computer skills (MS Office)

  • Valid Virginia Driver's license and proof of insurance

  • Ability to drive Agency Vehicle

  • Must have reliable transportation for client transport

  • Ability to travel as required

  • Must be highly committed

  • Needs Assessment Experience desired

  • Community Resource experience preferred
