Description ESSENTIAL RESPONSIBILITIES:  The Family Therapist provides family centered assessments and treatment and assist the case manager in treatment planning for the client.  This position is supervised by the Clinical Director and has no supervisory responsibility.  This postion provides flexibility in meeting the needs of the clients.  Appointments can occur after hours and/or on weekends, if needed. Duties include: 

 

·     Required to carry 5 cases and maintain weekly contact with those clients

.    Participate in the development of comprehensive individualized service plans.

·     Maintain a working knowledge of services that are appropriate for the needs of the person served.

·     Collaborate with case manager, psychiatrist and any other professionals involved in the treatment of the client.

·      Generate, maintain, submit, and review all relevant documentation in a timely manner, including (but not limited to) daily service tickets, service hour reports, incident reports, necessary diagnostic assessment forms.

Requirements

QUALIFICATIONS:            This position requires the following:

 

·                     Bachelor’s degree (Master’s preferred) from an accredited college and licensure in the state of Ohio as a LSW, LPC, LISW & LPCC

·                     Be at least 21 years of age

·                     Good written and verbal communication skills

·                     A valid driver’s license and current automobile insurance.

·                     A willingness and comfort level in providing community-based services to a diverse population of clients.

·                     Good problem solving skills and the ability to remain calm and professional in crisis situations.
