First Anniverary bonus equal to 7% of annual gross earnings after completion of 1 year.
Medical, Dental, Vision, RX and Pension Plan after 1 year of service equal to 15.9% of annual salary.
Position Summary:
Staff Pharmacist is responsible for the accurate identification, preparation and distribution of drugs, the monitoring of patient drug therapies, and the provision of pharmaceutical care to patients served. Pharmacists supervise and direct ancillary personnel (technician, interns) working in their immediate area. In the absence of a Pharmacy Supervisor, a pharmacist may be required to assume essential responsibilities and perform the duties of the Pharmacy Manager.
This position provides care/service to a critically ill through rehabilitating, neonatal through geriatric patient population in a manner that demonstrates an understanding of the functional and/or developmental age of the individual served.
Position Accountabilities:
1. Prepares and dispenses medication orders per physician request according to OVHD policy and procedures and protocols. Interprets medication orders (written and verbal) accurately. Transcribes orders to the computerized patient medication profiles accurately. Maintains accurate, complete patient profiles. Evaluate patient medication regimens. Screen for allergy information before dispensing medication. Screen for drug-drug/food-drug interactions. Screen for duplication of treatment. Appropriately doses medication according to patient’s renal function and age. Screens patient profile for allergy information, allergies are reviewed prior to dispensing medications.
2. Maintains drug formulary making rational drug therapy decisions. Sustains the formulary by minimizing non-formulary procurement, utilizing therapeutic substitution protocols, and promoting rational drug therapy selection. Provides clinical consultation and clarification to practitioners. Suggests appropriate cost-effective therapeutic alternatives to the medical staff, as needed. Initiates therapeutic interchanges according to protocol. Participates in the quality improvement and medication use review activities of the department.
3. Prepares and Dispenses IV admixters orders per physician request according to OVHD’s policy and procedures. Compounds and dispenses large and small volume parenterals accurately. Compounds and dispenses TPN accurately. Compounds and dispenses chemotherapy products accurately.
4. Contributes to the quality and effective operations of the department. Supervises and directs pharmacy supportive personnel. Verifies the daily activities of the pharmacy technicians. Works independently with minimal supervision. Organizes and prioritizes work assignments. Ensures pharmacy services are provided in a timely manner. Answers the telephone, identifying self and department. Directs calls to appropriate personnel. Answers requests at the window. Keeps pharmacy areas and equipment clean, neat, and well organized. Performs the essential duties of pharmacy management if management is absent. Fosters a team environment by providing orientation and training to new team members. Assists coworkers in tasks, as time permits.
5. Performs procedures for the appropriate control and storage of medication.
Completes inspections of all assigned medication storage areas at least monthly. Identifies and replaces outdated and unusable drugs. Maintains emergency drugs supplies appropriately.
6. Maintains competence required for current job title/position. Maintains current pharmacist licensure. Attends pharmacy staff meetings. Attends orientation, education, and training programs. Reviews literature and other materials pertinent to the practice of Pharmacy. Completes all competence assessment/skills lists as required. Dispensing systems. Laminar flow hoods. Bio-safety cabinets. Pyxis.
Unit-specific accountabilities
1. Ensures the safe, appropriate, cost-effective medication therapies for patients according to established policies, procedures and protocols. Reads, extracts and interprets information in patient charts accurately. Detects and reports suspected adverse drug reactions accurately and in a timely manner. Appropriately assess patients drug therapy for timely conversion of IV to PO medications.
