POSITION SUMMARY

Case Management is a collaborative process between the health care team to meet
the individuals health care needs through communication and available resources
to promote quality cost effective outcomes.

The Administrative Supervisor will supervise and give direction to the Post
Acute Case management team. This will include but not limited to RN Case
Managers, Discharge planners, and admissions clerks. The Post Acute Case Manager
will have input in hiring, training, disciplining, appraising, interviewing, and
termination.

POSITION REQUIRES:

Ability to supervises licensed RN UR/Case Manager, LVN Discharge Planner,
Admissions Clerk. Provides on the job direction including assignments, input and
coaching of employees. Provides input to hiring, training, disciplining,
appraising, interviewing, and termination. Assists in supervisory role in
Director�s absence. Graduate of an accredited school of nursing. Requires
current California Registered Nursing License. Baccalaureate degree preferred in
nursing or related health care field. Requires one-year experience in case
management in clinical practice in an acute care/Post Acute care setting.
