Description The Clinical Pharmacy Specialist is responsible for the provision of pharmaceutical care in a designated specialty area by working collaboratively with a multidisciplinary health care team to promote wellness, maintain current health, and/or intervene in acute or chronic illness. S/he works to promote the use of established medication use policies and assists with the identification and development of new policies within the Department of Pharmacy and the Pharmacy and Therapeutics Committee. S/he orients, mentors and serves as a clinical resource for staff and students. S/he provides education to patients and other health care practitioners and assumes a leadership role in the profession of pharmacy. S/he develops positive relationships in satisfying the needs of internal and external customers. S/he supports and participates in the institutional mission of providing quality patient care, education, and research.

1. Provides clinical pharmacy services in the designated specialty/patient care area.
2. Provides advice and guidance to medical staff and other clinical providers on the provision of drug therapies and drug protocols with regards to age-specific populations.
3. Provides patient specific information to patients and patients' family members on safe and effective drug usage.
4. Communicates with co-workers and individual patients/caregivers when requested or upon own initiative in order to resolve or prevent drug-related problems and to assure optimal patient cooperation in drug therapy.
5. Communicates and collaborates with the health-care team to ensure timely and accurate exchange of information across the continuum of care.
6. Reviews, interprets and enters physician medication orders into computer system. Accurately dispenses medication to patients.
7. Provides drug information to physicians and other professionals. Documents information and shares findings with other health care providers in a manner that reflects a positive image of a pharmacy professional.
8. Serves as a clinical resource in area of specialty, including on-call availability.
9. Prepares drug monographs and reviews drugs for formulary consideration.
10. Assists in planning, development, and maintenance of the formulary. Cooperates and assists in the periodic review of the formulary within the specialty practice area.
11. Promotes rational, cost-effective drug therapy. Communicates with pharmacy to avoid waste during preparation.
12. Assists Manager of Clinical Pharmacy Services in Medication Use Evaluation (MUE) and Adverse Drug Reaction monitoring.
13. Participates in other departmental quality improvement efforts as requested.
14. Actively and effectively participates in departmental and hospital committees as assigned.
15. Provides educational services to medical, pharmacy, and nursing staff. Actively pursues opportunity and participates in lecture, literature and other educational activities directed at medical and other professional staff.
16. Participates in the design and implementation of clinical competency assessments.
17. Serves as a mentor to pharmacy staff, aiding in the development of their operational and clinical skills.
18. Contributes to the medical and/or pharmacy staff in the form of publications related to practice.
19. Supports the mission of the Department of Pharmacy in the training of pharmacy students and residents. Serves as a preceptor in the area of specialty. Attends and participates in education-related committee meetings as assigned.
20. Supports drug research conducted by the institution. Participates in drug research protocols, and helps to assure optimal handling of investigational drugs and appropriate documentation when involved with a protocol.
21. Participates in and/or takes responsibility for projects and activities which support the goals of the department and hospital.
22. Performs other duties as assigned.
Requirements EDUCATION & TRAINING:
Doctor of Pharmacy is required
Completion of a Pharmacy Practice Residency is required;
A specialized pharmacy residency is preferred.

EXPERIENCE:
In lieu of a specialty practice residency, three years of practice experience in the area of specialty is required.
Experience in project or program coordination.
Leadership experience in planning and setting direction for a department, team or program is preferred.

LICENSE & CERTIFICATION:
Registered pharmacist or eligible for registration in Kentucky.
Board certification is desired.
