This position is in Home Based Primary Care (HBPC). Travel to the veterans' homes is required on a regular and recurring basis. This is within a 60 – 70 mile radius of Nashville, TN. Performs the initial and ongoing assessments of the veteran's functional status.

Must possess a valid driver's license and have a good driving record.
