Description Job Summary:
The Home Infusion Bench Pharmacist will provide pharmaceutical services to all Home Infusion patients and customers of CHI Nebraska HealthConnect at Home.  The Home Infusion Bench Pharmacist performs compounding and distribution of drugs and medicinal agents from the space allocated and pursuant to the terms of the contract established between GSHS Good Samaritan Hospital Pharmacy Services and CHI Nebraska HealthConnect at Home.  The position may perform other duties for the GSHS Hospital Pharmacy when not performing home infusion activities as specified by contract. The Home Infusion Bench Pharmacist is responsible for the proper storage and distribution of drugs and medicinal agents to the ultimate user; maintenance of proper records; and relating of drug information within the limits of professional judgment. The Home Infusion Bench Pharmacist has supervisory, administrative, technical and judgmental responsibility for assuring the appropriateness of patient medication regimens. This responsibility includes acquisition and preparation, and/or sterilization of medication forms as well as their distribution, control, inspection, storage, labeling and records maintenance. The position is also responsible for using research, interpretive, and writing skills to provide accurate and timely responses to drug information and consultation problems; assisting in the planning, organization, implementation, monitoring and enforcement of Home Infusion Pharmacy Services policies; participation in patient, student and staff teaching programs; participating in drug utilization review activities; performing clinical pharmacy services as needed; and accepting on-call responsibilities. Essential Job Functions:
  1. Reviews physician medication orders for appropriateness to identify possible dosing, scheduling, contraindication, allergy, drug interaction and incompatibility problems; contacts the physician if follow-through or drug order clarification is necessary; communicates with physician regarding the formulary suggesting changes when appropriate.
  2. Assumes responsibility for interpretation and verification of medication orders and preparation and distribution of standard and extemporaneous dosage forms in a timely manner.
  3. Demonstrates the knowledge and skills necessary to provide care based on the physical, psychosocial, safety and related criteria appropriate to the age of the patients served.
  4. Instructs patients on medications to be taken. Drug name, strength, indication, dose, interval, possible side effects, refill instructions and other information is conveyed.
  5. Assists in the enforcement of departmental policies and programs.
  6. Supervises the activities of the pharmacy technicians and specialists in all medication distribution functions.
  7. Maintains necessary records to fulfill Federal and State legal requirements and departmental policies regarding all phases of handling controlled substances and prescription items in the institutional setting.
  8. Monitors drug inventory and assists in reordering, and maintaining an adequate inventory.
  9. Performs related duties as assigned or requested including assistance with Hospice and Home Health patients as requested.

  1. Participates in professional pharmacy organization(s) at the local and national levels to be aware of pharmacy developments.
  2. Performs value-added activities which result in positive financial performance and customer satisfaction.

Requirements Education:
  • Graduate of an accredited college of pharmacy with a Bachelor's degree in Pharmacy.
  • Five years of college or university, of which at least three years must be in an accredited school or college of pharmacy.
Experience:
  • Must fulfill the requirements of the 1986 Nebraska Pharmacy Act (71-1), 142, which defines a pharmacist as one licensed in the State of Nebraska to compound and dispense medications and preparations upon the written or verbal order of a medical practitioner. 
  •  Minimum of 1 year hospital or compounding experience.
License/Certification: 
  • Nebraska Registered Pharmacist License required.
  • Must have a valid Nebraska driver's license and maintain automobile insurance coverage.
