 Description Includes, but is not limited to:

- Complete physical assessment with attention to cardiovascular, genitourinary, gastrointestinal, musculoskeletal, endocrine, hematopoietic, neurological, EENT, nutritional, baseline indicators, e.g., vital signs and functional needs of the patient.

- Establish and review initial and ongoing assessment information relevant to developing an individualized age/culture specific plan of care.

- Identifies, prioritizes and documents clear, measurable, realistic patient and family centered goals to ensure effective outcomes.

- Actively participate in scheduled supervision case management conferences.

- Assess/instruct regarding disease process, signs and symptoms of exacerbation, and appropriate coping skills and adaptive behavior.

- And other duties as required by the supervisor.

 Requirements

 

  1. Bilingual Spanish Speaking a plus.
  2. Comprehensive knowledge and skill in current nursing practice.
  3. Knowledge of the concept of team and primary care nursing and the implementation of both models of care delivery.
  4. Knowledge of the community resources.
  5. Ability to make meaningful observations, evaluations and reports.
  6. Ability to recognize the needs and concerns of patients, caregivers, and the multidisciplinary team to maintain therapeutic relationships.
  7. Ability to accept and constructively utilize supervision.
  8. Ability to provide skilled nursing care based on scientific principles, the nursing process, basic behavioral concepts, and intelligent professional judgment.
  9. Graduation from an accredited school of nursing. Possession of a current Connecticut license to practice as a Registered Nurse.
  10. Must perform other duties as required by the Supervisor.
  11. Valid driver’s license.
  12. Acceptance of variety of patient/caregiver lifestyles and ability to adapt to associated care delivery needs.
  13. Must have the ability of getting into and out of a vehicle 10-15 times a day.
  14. Must have excellent finger dexterity.
  15. Must have the ability to speak clearly.
  16. Normal range of vision is required with or without correction.
  17. Normal range of hearing is required with or without correction.
