- Graduate of an accredited physical therapy program at Baccalaureate degree level
- Graduate of an accredited physical therapy program at Masters degree level preferred
- Current state license showing registration as a licensed Physical Therapist
- Two years of experience as a Physical Therapist
- One year of experience as a licensed Physical Therapist in a home health setting preferred
- AHA CPR certification
- Physical within the last six months
- Two step PPD or Chest X-ray
- Two professional references
- Criminal history report with no activity listed
