Nurse Clinical Liason

We are currently searching for a Clinical Nurse Liaison.

The Clinical Liaison Nurse is responsible for creating and fostering positive relationships with all community referral sources of potential admissions to all programs including short-term rehabilitation, long-term nursing home care, respite care and home health care.  Referral sources include but are not limited to hospital discharge planners/social workers, physicians, nurse practitioners, hospitalists, geriatric case managers, and homecare agencies.

Education: Bachelor of Nursing Science Licenses/Certification: Valid driver’s license. Licensure as a Registered Nurse in New York State.  PRI/Screen Certification preferred Experience:  A minimum of three years of acute care / skilled nursing experience.  Two years of admissions/patient assessment/marketing experience preferred. Bilingual- English/Spanish a plus.(We are probably going to insert some job titles here.  We are getting the appropriate titels from some of our sources).

• Knowledge of New York’s PRI/Screen System.
