We are looking for FULL TIME Care Managers for a State Guardianship Program that services Gloucester, Salem, Cumberland and possibly Atlantic Counties for a 2-year commitment. The ideal candidate MUST have geriatric experience and we prefer a background in, or knowledge of, Medicaid, Mental Health and Guardianship. You should possess a Bachelor's Degree or Master's Degree with or without license. This position requires a 40 hour work week Monday-Friday approximately 9:00 AM to 5:00 PM. We do pay an additional $2.63 per hour for travel expense. The position also requires participation in the on-call rotation, roughly twice a month. On call shifts can include overnight (5:00 PM to 9:00 AM), weekends (Saturday 9:00 AM to Sunday 9:00 AM or Sunday 9:00 AM to Monday 9:00 AM) and holidays. They involve only telephone work – no actual field work. Pay is as follows: Bachelor's Level $17.50/hour Master's level $20.50/hour LSW $24/hour LCSW/LPC $27/hour. Duties and responsibilities would include: Visit assigned wards on a routine basis, with presumptive visitation schedule. Nursing Home Residents at least 4 times per year. Residential Health Care Facility or Boarding Home Residents at least 6 to 8 times per year. Home Residents at least every other week. Engage the services of professionals as necessary to appropriately meet the needs of the client, e.g. doctors, health aides, therapists, nurses, and other services related to maintaining the “care" needs of the individual. Initial assessment - must be completed as soon as possible following appointment of PG. With approval from OPG assigned attorney, the Care manager may conduct the initial assessment without the court order. Upon appointment, meet with the client as soon as feasible, address all issues of the client that require immediate action, and assess: Appearance Mental status Ambulation assessment Activities of Daily living Weight, diet, and appetite Skin assessment Continence Medical, nursing, social service progress notes Physician progress notes Diagnoses Medications and other medical orders or treatments Allergies to medications A list of key contacts including service providers and their contact information, a description of services provided to the client Psychosocial functioning Consultant progress notes Care plan and recommendation Client’s preferences with respect to Life Sustaining Treatment (LST) Client’s current living situation Client’s educational, vocational, and recreational needs Client’s current support system and needs not met Significant individuals and family members Entitlement program receipt; obtain copies of program documents (PAS, etc) Property issues Funeral planning Inventory of belongings Photograph of client for the OPG record Clothing needs, groceries, furniture, etc. Other significant findings Verify and gather identifying information including: SS# and date/place of birth Medicare information, insurance and medical coverage information, Medicaid status Gender Marital status, name/location of spouse Citizenship Ethnicity and primary language spoken Military status County of location, current address (and length of occupancy), former address, current phone number, client email address Income source and amount Primary physician name and number Funeral home name and number Other relevant data Communicate to the client the role of the guardian, explain the rights retained by the client and attempt to gather any missing necessary information regarding the client. After the first meeting with the client, notify relevant agencies and individuals of the appointment of a guardian. Provide a copy of the above assessments to OPG to be included in the client record.
