Registered Nurse (RN) - Home Health

JOB SUMMARY

Responsible for providing home care services consistent with the mission, vision and goals of the Agency.  The RN is responsible for providing skilled nursing services under a multi-disciplinary plan of care approved by the  patient's physician. Nursing care includes, but is not limited to, patient assessment, evaluation and coordination of the medical / nursing needs of the patient;  performance of prescribed medical treatments; and  instruction of caregivers in the provision of care to the patient.  The RN shall provide for the continuity of services for the patient and his/her family. 

DUTIES AND RESPONSIBILITIES

• Complete initial and on-going comprehensive assessments of the patient’s needs including OASIS assessments at appropriate time points.
• Identify the patient and family’s physical, psychosocial, environmental, safety and developmental needs and initiate an individualized patient plan of care based on the needs identified.
• Revise the plan of care as necessary.
• Provide service and /or treatments requiring substantial and specialized nursing skills.
• Initiate appropriate preventative and rehabilitative nursing procedures.
• Counsel, support and instruct the patient, family and caregiver in meeting nursing and related needs.
• Refer to other services as needed.
• Document nursing assessments, problems identified, appropriate goals, care plan,  interventions provided and evaluation of patient / family outcomes achieved .
• Coordinate all patient services.
