Assistant Director of Nursing (RN) Home Care

 

Job Purpose:

 

Southwest Home Health Care, a growing Medicare Home Health Agency in Houston is looking for an Assistant Director of Nursing with at least three to five years of home health management experience. The successful candidate will assist in overseeing all clinical operations and ensure that patients are cared for with excellence.

  

The Assistant Director of Nursing will assist in the following Duties...

 

* Assures quality of care by developing and interpreting standards of care; enforcing adherence to state board of nursing and state nurse practice act requirements and to other governing agency regulations.

 

* Maintains home health staff by assisting in recruiting, selecting, orienting, and training.

 

* Completes patient care requirements by scheduling and assigning patient care staff; following up on home care results.

 

* Maintains home health operations by initiating, coordinating, and enforcing program, operational, and personnel policies and procedures.

 

* Assists in growing and developing employees by coaching, counseling, and disciplining employees; planning, monitoring, and appraising job results.

 

* Establishes a compassionate environment by providing emotional, psychological, and spiritual support to patients, friends and families.

 

* Promotes patient's independence by establishing patient care goals; teaching and counseling patient, friends, and family and reinforcing their understanding of disease, medications, and self-care skills.

 

* Maintains documentation of patient care services by auditing patient records.

 

* Resolves patient care issues by working patient care staff to personalize patient home care assessments; collecting relevant information; conferring with patient care teams; assessing patient home-care needs.

 

*Keeps physicians informed of patient status by monitoring and reporting home care services rendered and/or modified; monitoring home care progress; documenting care visits; anticipating additional home care services needed, i.e., infusion therapy, wound therapy, physical therapy, psychiatric care and/or other specialty programs.

 

* Improves home care outcomes by studying, evaluating, and re-designing home care procedures.

 

* Serves and protects the home care agency by adhering to professional standards, home care policies and procedures, federal, state, and local requirements, and professional and licensing standards.

 

* Updates job knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations.

Requirements

Skills/Qualifications:

 

Judgment, Supervision, Communication, Nursing Skills, Creating a safe, Effective Environment, Medical Teamwork, Managing, Training, and Developing clinical Staff. Active MIchigan Registered Nurse Licensure.
