Position Summary
Responsible for providing nursing services and education within the scope of practice as set forth by the North Carolina Board of Nursing to individuals and their families in a homecare or a facility setting. Coordinates plan of care by working with interdisciplinary team to anticipate and meet patient needs. Serves as the RN member of interdisciplinary team, to meet specific plan of care goals. Coordinates closely with related disciplines to ensure professional service delivery. May be asked to provide oversight to NAs, LPNs, volunteers, etc.

Registered Nurse - RN
Education and Experience
A Diploma or Associates Degree in nursing required; Bachelors Degree preferred. Current, active, and unrestricted NC license as Registered Nurse (RN). Current CPR required; CHPN preferred.
Minimum 1 year related experience for LDHH and 2 years for other nursing positions. Experience in direct care of patients with life limiting illnesses required. Related experience may include hospice and palliative care; family medicine; internal medicine; geriatrics, oncology; home health care; medical surgical.
Computer proficiency is required.
